Peritoneal Dialysis

We proudly accept and care for Peritoneal Dialysis patients. Our nursing team receives annual training in order to provide better patient care and improve quality of life.

Frequently Asked Questions

What information is needed to help the referral process go smoother?

-Nephologist Name, Name of the PD Clinic, and Family Contact Information.

-These are required before we can proceed in the review process.

Can PD residents also receive therapy services?

-PD residents can receive skilled therapy during their stay at Hillview CLC.

Are you able to do manual exchanges?

-Each case is reviewed by our clinical care team to ensure we can meet the patient's needs.

If new to PD, what happens first?

-A PD Clinic will need to be chosen by the resident or their Responsible Party. Once they have a signed agreement with the clinic, the PD Clinic will be able to order a cycler and supplies as well as send us the order for exchange. We recommend choosing a clinic close to the resident's primary residence so they can continue services after discharging from our community.

Upon admission, will they continue to go monthly to the PD Clinic?

-Yes, this will be coordinated by family members, our social worker here at Hillview, and the PD Clinic.

Do you provide education on using a cycler?

-Our staff has the ability to provide one-on-one training with the resident or their family for at-home care.

​

What specific services are offered for PD residents?

-Our nurses provide proper site care, monitor vital signs and weight changes, assess for signs of infection, monitor fluid and electrolyte balance, and create a schedule that offers minimal inconvenience for the resident.
